To complete the Nameless Hospital endings in NTE, you need to collect clues and insert them in the cameras’ room, on the second floor. The first floor contains the clues that get the first ending, the second floor contains the clues that trigger the second ending, and the third floor has the clues that get the third ending. However, the fourth ending is more complicated, as you need to be more involved in understanding the Nameless Hospital’s lore.
All Nameless Hospital Endings In NTE
You can get all four endings of the Nameless Hospital by entering its nightmare four times. You can target a specific ending each time, as you gradually learn your way around. During my runs, I found a way to quickly get the clues needed for the endings without interacting with many of the Nameless Hospital puzzles. For example, these runs won’t include activating the generator for power. However, I’ll still explain how to activate the generator at the end of the guide.
Here’s how to get every Nameless Hospital ending in NTE:
Nameless Hospital Ending 1: Collect three Anomaly Disorder clues.
Nameless Hospital Ending 2: Collect three Surveillance Record clues.
Nameless Hospital Ending 3: Collect three Anomaly Experiment clues.
Nameless Hospital Ending 4: Collect one hidden clue and two specific Anomaly Experiment clues.
Nameless Hospital Ending 1
Getting the first Nameless Hospital ending in NTE is pretty easy, as all you need to do is collect three Anomaly Disorder clues. The first one is exactly at the desk in front of your starting position.
Get out of the room, and keep following the corridor.
Do not enter any room, and follow the corridor to the right side.
There, you’ll reach the first-floor lobby, where you can get the NTE first-floor key in Nameless Hospital, called Keychain: 1F.
From there, go to the end of the lobby.
You’ll find a door on the corner.
Inside, you can get the second Anomaly Disorder clue in Neverness to Everness.
Now, head back to the lobby and go through the corridor again.
This time, keep moving forward until you reach the first door after a corner.
Open the door and head inside to get the third Anomaly Disorder clue in Nameless Hospital. Be careful, as after taking that clue, a mummy-type monster will appear and start a hide-and-seek type of game. It doesn’t take damage, and if it catches you, you’ll spawn at the starting point. I didn’t find it as dangerous as it was annoying, since you’ll have to walk all the way back to the area where it caught you.
Do not go back to where you came from. Instead, continue to the next room and exit from there into another lobby.
Follow that lobby to the end without going into any room on the right side.
You’ll eventually reach a room at the end of the corridor, with a door on its left side.
Go into the new room, and you’ll find a yellow screen that has the fourth Anomaly Disorder clue. A bonus clue, if you missed or didn’t find the previous ones during this Nameless Hospital run.
Continue moving to the next room to find a room with a blocked door and a cabinet to its right.
There, you can find the key to the second floor, called Spare Key: 1F.
Remove the trash blockage, then leave the room to find the gate that leads to the second floor.

How To Switch Appraiser In NTE (Neverness To Everness)
Players can change the Appraiser after completing a quest in Neverness to Everness.
Take the stairs, and you’ll reach the second-floor main lobby immediately. There, behind the main desk, you can enter the room where you’ll find three cameras.
You need to approach the cameras and insert the Anomaly Disorder clues you collected.
After placing the clues, you will get Ending 1, which will finish the first Nameless Hospital run in NTE.
Nameless Hospital Ending 2
From now on, when you enter the Nameless Hospital nightmare again, you’ll always start with a Staff Elevator Card, which is the key you need to activate the elevator.
To get the second Nameless Hospital ending, exit the room you start at, and follow the room the monster enters.
It’s the room in front of the elevator.
At the end of that room, you’ll find a small desk that has the first Surveillance Record clue in this Nameless Hospital run.
You can continue moving forward, discovering more things, such as how to activate the generator in the Nameless Hospital. However, from my experience, heading back immediately to the elevator and going to the second floor is a much faster way to get Nameless Hospital Ending 2.
From the second floor’s main lobby, head to the corridor at its end.
Before you reach the lobby, a bunch of monsters will spawn.
After many Nameless Hospital runs, I noticed two types of monsters spawn here. Based on which type appears, it determines what happens on the second floor. If they are werewolf-looking monsters, you’ll get a second floor with rooms that react based on whether the lights are on or off. In that case, activating the generator becomes important. If they are floating creatures, the entire second floor will flood with water, and some doors will open without a key.
In both cases, you can get the Nameless Hospital Ending 2 no matter what happens on the second floor. After defeating the monsters, continue forward to the end of the corridor.
Go left, targeting the first room after the bathroom.
You can find its door right after the bathroom, on the same wall.
Go inside, and you’ll find the second Surveillance Record clue in NTE.
Get out and keep following the same corridor, on the same wall, to find the second floor’s generator room.
Inside, on the wall, you can spot the third and last Surveillance Record clue.
Now, you have all three Surveillance Record clues, and you’re ready to complete the second Nameless Hospital run in NTE.
Head back to the cameras’ room and insert all three Surveillance Record clues.
This will complete this Nameless Hospital run, and you’ll get to see the cutscene for Ending 2.
Nameless Hospital Ending 3
To get the third Nameless Hospital ending, immediately go to the third floor from the elevator.
Once you reach the Nameless Hospital’s third floor, you’ll find two locked gates, with nowhere to go. Luckily, the keys are just next to each gate. Target the door on the right side. You can find a clock nearby, with the key hidden inside it.
Open the door, and move to the right side to open a new room.
There, you need to find a key hidden in the front lockers before the timer runs out, or the room will kick the character out.
Take the key and head out to the door on the other side.
Open the door to get into a corridor. Take your right to find another room immediately after.
This room also has a timer, where some weird things happen while you’re inside it. Move to the other side of the room to find a file with the first Anomaly Experiment clue in NTE.
Get out of the room from the other side to enter another room, which has a blocked door. Destroy the blockage and exit to find another door in front of the one you just used.
Inside that Nameless Hospital room, some monsters will spawn.
Defeat them and investigate the second Nameless Hospital Anomaly Experiment clue.
Go back to the elevator corridor and open the other gate. You can find its key in a canister right next to it.
Head inside and open the door on your right.
It will lead to a blocked corridor, with two rooms on each side. Enter the one on your right.
You can find the third Anomaly Experiment clue inside.
That should be three Anomaly Experiment clues in NTE, and enough to complete this Nameless Hospital run.
Head back to the elevator and go to the second floor.
Enter the cameras’ room and insert all three Anomaly Experiment clues in NTE.
This will give you ending 3, and you’ll complete the Nameless Hospital run for the third time.
Nameless Hospital Ending 4
While the first three endings are somewhat easy to achieve, the final ending is the hardest, as it requires a lot of reading and researching between the clues. Of course, I’ll try to make it as simple as possible with this Nameless Hospital guide.
First and foremost, you need either the main character or Nanally in the team.
Start the last Nameless Hospital run by heading straight to the elevator, aiming to reach the second floor.
From there, follow the corridor after the main lobby area.
Keep going forward until the end of the corridor.
Take your left to target the room after the bathroom.
Again, it’s the room on the same wall.
This time inside, you can find a board on the right side of the wall. It will be either hidden or visible, based on the variation you get when you spawn the monsters before reaching the corridor.

If you get the flood variation, it will be visible. However, if you get the light-reactive variation, you need to interact with the switch next to the door to turn off the light. This will reveal the board.
Now, approach the board and interact with it to get the keys, called Keychain: 2F.
Go back to the start of the corridor, where you can find a blocked door. You can access that blocked room through the room right next to it. All you need is the key you just got.
If you get the flood variation, you can head straight to this room, as you don’t need a key to open it.
Inside that room, you can find a giant hole in the wall that leads to the previous blocked room.
This is where it gets interesting. I have tried countless times to reach the room above this one, as it holds the most important secret in the Nameless Hospital nightmare. Unfortunately, nothing worked but some kind of cheesing my way up there. You can easily do that by finding a high point for the main character to stand on.
From that high point, use their Skill and wait until they launch themselves in midair.
Once they do, instantly switch to another character and jump twice in midair, trying to reach the floor. It can only be done with the main character or through Nanally’s zero gravity movements.
Once you reach the higher room, interact with the middle sealed canister to get a Jigsaw, which is the most important piece in the Nameless Hospital nightmare. After getting the Jigsaw, there’s nothing else to do there besides opening the safe in NTE. Go back to the second floor and use the elevator to go to the third floor.
On the third floor, from the elevator, target the gate on the left side.
Enter the room and open the door to the right.
Go to the room on the right side.
Inside that room, you’ll find another Jigsaw that you need.
Go back to the previous room and insert both Jigsaws on the middle table.
That will spawn some monsters and create new doors and corridors.
Use the door in the back to enter a small room with some sealed canisters inside. From that room, go through the wall on the right side to reveal a hidden room that appeared when you inserted the Jigsaw.
Go to the desk and read the note there. You need to read it four times, as each time the text will change alongside the room.
After the fourth time you interact with the note, you’ll get the V???’s Note clue, which is the first clue to complete the fourth Nameless Hospital ending in NTE.
Now go back to the corridor where you found your second Jigsaw and move forward until the end.
Just before reaching the end, you can find a door on the right side, at the start of the corner.
Interact with the files on the desk to unlock the Anomaly Experiment Clue III, which is the second clue you need to complete the last Nameless Hospital run.
Right after you leave the room, enter the room in front of it.
At the end of that room, you’ll come across Mysty, who is an experimental object that can transform into other objects. Ask it to come with you and get out of the room into the corridor.
Go to the left after leaving the room and keep following the corridor.
Take a right turn and keep moving forward.
Just before going full circle, you can find a room there.

Completing the switch puzzle will unlock a hidden chamber containing the third and final clue for Nameless Hospital ending 4 in NTE, called the Anomaly Experiment clue VI.
Now, go back to the cameras’ room, on the second floor.
Insert the clues to complete the fourth Nameless Hospital ending in NTE.
How To Activate The Nameless Hospital Generator In NTE
To activate the Nameless Hospital generator in NTE, you need to follow a pattern. Activate the middle generator in the back first. Then, activate the one in front of it. Then target the generator at the end of the room, before activating the last one next to the door.
